Emergency Contact Cards

Here are two emergency contact information cards that you can print, fill out and keep in your wallet or boat. Make sure they can be quickly found if needed.

The card provides first responders and law enforcement with your name, address, phone number and other basic information. If you have medical conditions that need to be communicated or want to list your primary care physician, write on back of the card and where it says "Allergies" put the words "ON BACK".

The cards are slightly smaller than a business card when trimmed to size and should fit any wallet. To make these last, print on heavyweight paper, card stock or regular printer paper and glue to a business card.

We hope you never have to use these cards but better safe than sorry!
